This thesis examines patients’ access to and use of text-based e-consultations with their GP through a mixed methods design. Study 1 is an interview study with older patients, exploring their experiences with the service. Study 2 is a national survey among users, examining patterns of use. Study 3 is a free-text analysis exploring users’ suggestions for service improvement.
The integrated findings indicate that access to text-based e-consultations is conditional and depends on patients’ abilities and service characteristics. Use is shaped by both perceived convenience and constraints within the wider GP system, and text-based e-consultations operate both as a replacement for and a supplement to physical consultations.
Overall, findings show how the use of text-based e-consultations coexists with uneven access. Patterns of use are diverse and linked to patients’ needs and the availability of other GP services. Maintaining non-digital, dialogue-based GP service options is essential. Caution is warranted to address uneven access and to ensure that the use of text-based e-consultations does not pose clinical risks or increase GP workload.
